Application of the HCF has been scrutinized and it is observed as under:-
1) The HCF has not attached copy of valid authorization under the Bio Medical Waste Management Rules, 2016.
2) The HCF has not submitted any document regarding ownership like partnership deed / proprietorship certificate.
3) The HCF has not submitted any undertaking regarding bed capacity.
4) The HCF has not submitted building plan approved by the Competent Authority.
5) The HCF has not attached analysis report of combined chemical & domestic effluent.
6) The HCF has not submitted any document regarding installation of treatment facility.
7) The HCF has not submitted approval of municipal authorities for connectivity of MC sewer.
8) The HCF has not submitted revenue record of the land.
9) The HCF has not submitted detail of consent fee.
Kindly clarify within 02 days.
