CTOA/Renewal/SAS/2019/8693587 dated 22.01.2019, valid till 30.09.2023 for 100 bedded hospital with certain conditions mentioned therein.</span></p>

BMW/Renewal/SAS/2022/19977077 dated 24.11.2022 for 100 beds, valid till 31.03.2023 under the new name of MY Hospital Super Speciality Care (A Unit of Chandigarh Healthcare Pvt. Ltd.)</span></p>

Ltd.) for 100 bedded hospital alongwith requisite documents.</span></p>
<p class="MsoNormal" style="text-align: justify; text-justify: inter-ideograph;"><span style="font-size: 12.0pt; line-height: 115%; font-family: 'Cambria','serif'; mso-ascii-theme-font: major-latin; mso-hansi-theme-font: major-latin; color: black; mso-themecolor: text1;">Earlier, the hospital has deposited Rs. 50,000/- vide R. no. 19/4882 dated 13.11.2019 under Water (Prevention & Control of Pollution) Act, 1974, which was adequate upto 13.11.2024.</span></p>

The project proponent has submitted NOC for sewer connection vide GMADA letter no. 2570 dated 05.06.2012 in the name of Chandigarh Health care Pvt. Ltd. The project proponent has not submitted Memorandum of Association. </span></p>

Ltd. is running a hospital in Mohali in brand name of Mayo Healthcare Super Speciality Hospital since 2014, however, on the direction of Delhi High Court, mediation and conciliation centre New Delhi vide mediation petition no. 131/2020 dated 06.08.2020 and agreement between Mayo Foundation for medical education and research and Mayo Hospital Super Speciality hospital was signed. The Board of director of Chandigarh Healthcare Pvt. Ltd. in his meeting dated 07.09.2022 has decided to change the brand name of the hospital as My Hospital Super Speciality Care instead of Mayo Healthcare Super Speciality Hospital. The treated waste water is disposed into MC sewer. The reading of the water meter provided at outlet of STP was 2277.4 m3. As per record checked the reading is the same as on 14.10.2022, hence signifying that no wastewater has been disposed off from STP. The hospital has maintained record of operation of STP till 14.10.2022 only and the representative informed <span style="mso-spacerun: yes;"> </span>that due to very less generation of effluent due to very less staff left in the hospital and very few patients the STP is not operated regularly.</span></p>
