1. The institute has not submitted Partnership Deed/Proprietorship/MOA.
2. The institute has not submitted layout plan/building plans approved by Competent Authority.
3. The institute has not submitted arrangement made for treatment of domestic effluent as well as its disposal arrangements.
4. The institute has not submitted the detail of staff.
5. The institute has applied for renewal of consent to operate with a fresh OCMMS ID, however, the institute has not attached compliance of previous granted consent to operate/NOC as well as copy of previous granted consent to operate/NOC.
6. The institute has not deposited/linked consent to operate/consent to establish fee with the application.
7. The adequacy of the consent to establish/consent to operate fee will be checked after the compliance of observation no. 5 & 6.
8. The institute has not submitted the arrangement made for the treatment and disposal of Municipal Solid waste.
