| Clarification Note: |
On scrutinizing the application of the HCF it was observed as under:
1. The HCF has not submitted approved Site Plan/Location Plan of the Healthcare Facility.
2. The HCF has not submitted Land documents such as Registration deed/Jamabandi deed/Rent deed/ lease deed indicating details of property.
3. The HCF has not submitted Proof of date of Commissioning of the Healthcare Facility.
4. As per clarification point no.3, the adequacy of fee cannot be adjudged at this stage.
5. The HCF needs to clarify the reason for not applying for consent to operate under the Water Act, 1974.
You are therefore requested to submit reply of the above said observation within 2 days.
