| Clarification Note: |
The case of the HCF has been scrutinized and it was observed as under : -
1. The HCF has attached the NOC from EO, MC, Mansa in which the name of HCF is mentioned as Domeli Surgical Centre whereas the application has been applied in the name of DOMELI HOSPITAL & SURGICAL CENTRE.
2. The HCF has not deposited/linked the consent fee.
3. The HCF has not mentioned HCF commission date in the declaration so as to calculate the consent fee adequacy.
As such you are requested to remove above said observations and deposit/ link the authorization fee within 2 days so that the application can be processed further.
