Sir, It is submitted that the application of the HCF has been scrutinized and found that-
1. The HCF has not filled any details regarding bed capacity as well as water treatment and disposal details in the application form.
2. The HCF has not submitted any document with online application i.e., Copy of agreement with bio-medical waste treatment facility, Annual Report in form-4, ETP cum STP installation status report etc.
3. The HCF has not deposited / linked consent fee with online application.
As such, if approved, this application of the HCF may be returned herewith the request to re-apply the same after attending the above said observations, please. |
