The application was scrutinized and it was observed as under:
1. The fee submitted by the HCF has not been linked with the applications under the BMW Rules, 2016.
2. The HCF has attached 0 no. dental chair agreement with CBWTF, however, as per application form; there are 02 no. dental chair, which need to be amended/clarify.
As such, you are requested to kindly link the fee with the application and clarify the above said observation within 02 days so that the application can be processed further.
