| Clarification Note: |
The application of the HCF has been scrutinized and following discrepancies were observed:-
1) The HCF has not uploaded/ submitted the agreement carried out with the M/s Rainbow Environments (P) Ltd., Village Baliyali, Tehsil Kharar, Distt. Mohali, Site Plan, Location Plan, etc.
2) The HCF has not deposited any authorization fee under BMW Rules, 2016.
3) The HCF has submitted Manifest receipt in which PHC Behrampur has been mentioned and on the highlighted receipt mentioned name is not readable.
4) The HCF has not filled the application form properly w.r.t the details regarding medical treatment provided to the patients not mentioned.
As such, it is therefore, requested to upload document/ give reply to the above said points within 05 days so as to enable this office to proceed further in the matter.
