The clarification was raised with respect to following discrepancies:-
1) The HCF has not uploaded/ submitted the agreement carried out with the M/s Rainbow Environments (P) Ltd., Village Baliyali, Tehsil Kharar, Distt. Mohali, Site Plan, Location Plan, etc.
2) The HCF has not deposited any authorization fee under BMW Rules, 2016.
3) The HCF has submitted Manifest receipt in which PHC Behrampur has been mentioned and on the highlighted receipt mentioned name could not be readable.
4) The HCF has not filled the application form properly w.r.t the details regarding medical treatment provided to the patients not mentioned.
The HCF has failed to submit the reply/ document of the above said discrepancies for clarification raised within stipulated time. As such the application could not be processed at this moment and is returned herewith with a request to resubmit the application after attending the above said observations.
