| Clarification Note: |
On scrutinizing the application, following observations are raised:
1. The hospital was earlier granted authorization under Bio-Medical Waste Management Rules, 2016 for 57 beds. However, the hospital has now again applied for fresh authorization under Bio-Medical Waste Management Rules, 2016 for 57 beds. The project proponent needs to apply for renewal of authorization under Bio-Medical Waste Management Rules, 2016.
2. The hospital has not submitted analysis report of effluent samples carried out by Board’s/ approved lab.
3. The hospital has not submitted Annual Report on Bio-Medical Waste Management in Form-IV, as per special condition of earlier granted authorization.
4. The self-declaration as per Annexure-1 submitted by the hospital is not proper as the same is not attested by the authorized person and no detail mentioned regarding the treatment of liquid waste in ETP-cum-STP installed by the hospital.
You are therefore, requested to reply the above said clarification in 1 day. |
