The application has been scrutinized and it is observed as under:
1. The HCF is required to submit proof of commissioning of the clinic at the proposed site.
2. The HCF has mentioned its category/type as Government in the application form, whereas the agreement executed with the Common Biomedical Waste Treatment Facility indicates the HCF is a private entity.
In view of the above, the HCF is required to submit the above documents and reply within three (03) days positively, in order to further process the application.
